Make a Payment »CHADIS Pre-visit Questionnaires
CHADIS is a web-based screening, diagnostic and management system that administers and analyzes pre-visit, online questionnaires completed by parents, teens or teachers and provides clinicians with instant access to valuable clinical data and resources.
CHADIS surveys are an integral part of the visit as we can obtain information to provide the highest quality of care. If you elect not to complete these required questionnaires we will be unable to adequately assess your child and do not feel comfortable providing care, as this does not follow AAP guidelines. If you choose not to complete the questionnaires, another provider may be a better option for your family.
Most insurance companies cover most CHADIS questionnaires; however, it is the patient’s responsibility to verify coverage with their insurance company. If any questionnaires are denied by insurance, the cost will be the patients responsibility.
Beginning 11/09/2020, all CHADIS questionnaires will need to be completed 24 hours prior to the scheduled appointment time. Failure to do so will result in having to reschedule the appointment.

Telemedicine Visits with Anytime Pediatrics
Your child’s next Fairhope Pediatrics visit could take place in the convenience of your home, using the Anytime Pediatrics telemedicine app.
Insurance Coverage and Cost
Telemedicine visits may be covered by some insurance providers, including Aetna, Alabama Medicaid, Blue Cross/Blue Shield of AL, and Cigna. Ask your insurance provider if telemedicine is covered by your plan.
If your plan does not include telemedicine visits, there will be an out-of-pocket cost of $50 per visit.

Once we receive your forms, we will create an Anytime Pediatrics account for you. Watch your inbox for a welcome email with your account information. (These messages can sometimes be marked as spam, so check your junk mail folder as well.)
Telemedicine Appointments
Setting up a telemedicine appointment is easy, just follow these steps:

- Launch the Anytime Pediatrics web or mobile app fifteen minutes before your scheduled appointment.
- If you have photos that the doctor might like to see, please upload them using the app before your appointment begins.

We are required to report visits outside of normal business hours to your insurance provider. Appointments scheduled before 8am, after 5pm, and on Saturdays will incur an additional fee, which may be applied to your copay, coinsurance, or annual deductible.
